kitchen appliance valve/hose leak

I had a leak behind kitchen cabinets. A plumber said this was coming from the washing machine valve. He replaced the separate washing machine & dishwasher valves with a dual tee valve. He didn’t replace the washing machine inlet hose attached to the valve. After this work was done we found a new leak at the other end of the hose attached to washing machine even though the washing machine had not been used. This hose was then replaced and seemed fine for a while. Now the original leak problem at the dual tee valve is happening again. Not sure if this leak is due to the valve or the washing machine hose nut but these problems have been going on for months. The leak is a slow drip. What needs to be replaced now and could some other problem be causing these continued leaks?
