Water leaking under door threshold, causing skirting damage. Ideas?

I had a new composite door fitted a few years ago. It has a metal threshold and a upvc external sill. I noticed that when it rains, the skirting board running into the inside of the door frame on the opening side gets damp and has now swollen. I have been trying to understand where the water is coming from and have now been able to see that it is actually coming inside from the underneath of the threshold. The tile grout in the floor that meets the back of the threshold has started to come away as a result.
I have checked the drainage holes in the threshold operate as they should. I wondered if the fixing screw holes in the threshold should have silicone or a washer round the screw? Also, the builder left us with a big gap below the sill that needs mortar put back. Although this is on my to-do list, I don't think this is to do with the leak as it would be lower outside than the internal water ingress. Does anyone have any ideas?

This is one of those “I would need to see it to better understand” unfortunately but the “big gap” is the most likely issue. Rain doesn’t hit the ground and stay there. splash back can cause damp. Plus water will naturally find anything it can bind to and work its way in. It needs to have the mortar set in then see if it stops. Could be a lot cheaper than a door fitter coming to tell you the whole sill needs replacing
